Background Check
These forms must be filled out by anyone 18 and older prior to serving in Shabbat School. This is a 2-page form. Sign the first page, Criminal History Consent Form only in the presence of a notary. The second page, Georgia Bureau of Investigation Consent Form, does not need to be notarized. Bring both forms and a copy of your driver's license to the notary.

Student Registration
All parents must complete a registration form for each child.

Parents must complete this form if your child is between the ages of 13-17 years old, and wishes to assist in Shabbat School.
